Telehealth information and acceptance of Terms
An explanation of the relationship:
In order to establish a doctor/patient relationship with us, you must see us in our office in person. If you do not see us in our office in person, there will be a limited doctor/ patient relationship between us and you. We call these communications “Telehealth services.” These service we provide to you will consist of informarion and suggestions that are intended to assist you and your health care provider in using natural means to support your health. Telehealth are not intended to serve as a medical diagnosis or treatment of any kind.
Risk and benefits of telehealth/education services
By signing this document, you acknowlege that you understand the limitations of this manner of consultation ad there will be no proper patient/doctor relationship and Dr C Eybers cannot do a physical exacmination. You understand that a one-on-one consultation will be to your own benefit as she will be able to examine you and also prescribe recognised allopathic medication.
Confidentiality and security measures:
Although every reasonable measure will be taken to protect your information, in signing this agreement, you understand that your participation in telehealth services does make your information vulnerable to data hacking. Your informarion will be stored in two locations:
1. As hard copy in your patient file that will be locked up in our storage facilities.
2. As data on our computers. We apply the highest standard of internet security at all times and only qualified and appointed staff will have access to the information for the purpose of assisting Dr C Eybers in diagnosis, treatment and ongoing patient education purposes only.
You expressly consent to the transmission of personal health information between you, us and possible to third-parties where reasonably necessary and after obtaining your written consent, by using e-mail, telephone, video, cloud document storage and retrieval or snail mail in accordance with the Protection of Personal Information Act 2013.
Your consent to be recording of tele-intervies between you, Dr C Eybers and support staff and note that these recordings will be protected with the same diligence as all other information.

Our fees for telehealth services will be discusses with you when we first make contact.
